What is fractional laser?

Fractional ER: Glass 1540nm is a fractional non-ablative laser system that works by emitting light in a precise pattern of narrow, focused micro-beams, thereby creating a controlled injury in the skin. This triggers a natural rebuilding process in which new connective tissue forms in the treated area. The result is a fresher and younger appearance, with skin that has improved glow and surface texture.

Fractional laser can advantageously be used for skin rejuvenation, minor wrinkle reduction, and mild skin tightening, as well as for all types of scars, including flat (atrophic) acne scars, stretch marks, surgical scars, and burn scars.

At Apex, we also use fractional Pico laser as an alternative to the 1540nm laser for the same indications. These two different laser systems have the same effect but use different mechanisms. The 1540nm system (described here) is a warm laser, while the Pico system is a cold laser. The Pico system has an advantage for darker skin types and for patients who experience acne outbreaks after treatment with the 1540nm laser.

A typical treatment course consists of about 3–6 sessions with an interval of at least 4 weeks between treatments for optimal results.

Which areas can be treated?

We treat with fractional laser both on the face and body.

How does the treatment work?

The treatment can be performed with local anesthesia (numbing cream). You should arrive 45 minutes before the treatment to allow the anesthetic to take effect. Local anesthesia is not necessary when treating a few small scars.

If you wish to apply the cream yourself at home, you can purchase a local anesthetic cream over the counter at the pharmacy. Apply a thick layer of cream; the white color should be clearly visible. The area with the anesthetic cream is covered with cling film (“Wita wrap”), which enhances the effectiveness of the numbing.

The treatment itself feels like a prickling warmth, which can be felt in the skin for several hours afterward. You may take 1 gram of paracetamol about 1 hour before treatment to reduce any discomfort.

We always recommend the use of sunscreen both before and after treatment with SPF 50+ including UVA and UVB filters.

What is the effect of the treatment?

The treatment effect depends on the individual’s ability to regenerate new connective tissue. The effect of the treatments is generally good, and a long-lasting effect is expected, although it diminishes with the natural aging process. We always recommend maintaining the result with an annual follow-up treatment.

It is important to set expectations before treatment. Treatment fees are non-refundable.

Who cannot be treated?

  • Pregnant women

  • Relevant allergies

  • If you have tattoos or permanent makeup in the treatment area

  • If you have keloid formation

  • If you have an active skin disease or skin cancer

  • If you take medication or herbal remedies that cause photosensitivity or coagulation disorders

  • If there is an active infection in the area, including active herpes

Possible side effects?

The skin becomes red and warm immediately after treatment, often feeling like a mild sunburn. Some patients may experience some swelling, especially on the face, where significant swelling around the eyes can occur. These symptoms disappear within a few days to a week. Ice packs can be used on the treated area for 10 minutes every hour during the first 4 hours if needed.

In some cases, acne outbreaks can occur in the treatment area. For those few who experience this, preventive use of salicylic acid peeling directly after each laser treatment can be applied. For side effects related to peeling, see the patient information about peeling.

For repeated acne episodes after treatment, a switch to Pico laser treatment is offered (see separate patient information for this).

Avoid vitamin A creams and peelings for up to 1 week after treatment unless these are prescribed as part of the treatment plan.

Sauna, intense physical exercise, chlorinated water, and swimming in the sea are also discouraged during the first 24 hours.

Besides the immediate side effects already mentioned, rare cases of burns with wounds after treatment have been observed, and in severe cases, permanent scarring can occur in the treated area—though this is extremely rare.

In very rare cases, tiny pinpoint marks in the skin and discoloration corresponding to the laser insertion sites may persist for many months.

New skin is formed, so it is important to avoid sun exposure for at least 1 month after each treatment to avoid reduced treatment efficacy and pigment disturbances.
